The CHORUS mobile application is a companion to the CHORUS healthcare analytics solution designed to help clinicians treating patients with chronic illnesses in a number of therapeutic areas. Utilizing data already entered in your clinical practice's Electronic Health Record (EHR), CHORUS continuously calculates and reports clinical quality measures, identifies gaps in care, and produces simple but powerful patient summaries that identify patients by severity of illness and risk.
